Your Care Manager will:
Evaluate your unique situation and develop a customized care plan to perfectly address your needs
Coordinate care that encourages independence, yet address any safety issues
Accompany clients to physician appointments and provide families with details and important information
Assist with communication between clients, their families and their healthcare providers
Oversee the care plan and act as a local safety net for loved ones living far away
Make arrangements for, and coordinate the delivery of, in-home services:
– Caregivers
– Medication
– Meals
– Housekeeping
– Lifeline health alert and other assistive devices
– Installation of grab bars or other medical equipment
Provide families with trusted information on care options and the benefits of home care, assisted living and nursing homes

Dawn Moore, RN, CMC – Advanced Aging Life Care™ Professional
Dawn Moore has lived in Lee County most of her life. She has worked as an Aging Life Care Professional® through Lee Health’s Senior Care Choices for over 25 years. Dawn is an active participant in local and state Aging Life Care boards, and was honored as Outstanding Florida Chapter Member for the Aging Life Care Association in 2016. Dawn is also an active member of the Advisory Council for the Alvin Dubin Alzheimer’s Resource Center, and serves on boards for the Southwest Florida Coalition for Optimal Behavioral Health and Aging and SWFL Conference on Aging.
Tamara Mann, Aging Life Care™ Professional
Tamara Mann is a lifelong resident of Lee County and a graduate of Florida Southwestern State. She has over 27 years of healthcare experience. Geriatrics has always been her career focus as she feels it is essential to assist seniors in maintaining their highest level of function and preserving their personal definition of quality of life, no matter their limitations. She finds it rewarding to provide ongoing support to clients and their families in time of need.
